What Is Therapeutic Life Story Work (TLSW)?

Therapeutic Life Story work involves a qualified therapeutic practitioner guiding the young person through a coherent written narrative of their life history while integrating therapeutic techniques to support them to heal from past trauma and current psychological difficulties.

TLSW is a psychodynamic therapy and can therefore incorporate elements of several different therapeutic approaches, such as Cognitive Behavioural Therapy, Acceptance and Commitment Therapy and Play Therapy. Therapeutic qualifications and additional skills enable the practitioner to therapeutically explore the complex emotional, psychological and behavioural needs that some young person present with. Like LSDW, TLSW is a planned and structured process that includes the practitioner having access to information about the young persons history and creating a written narrative. This written narrative forms the basis of therapy sessions.

Some of the core practices include restructuring of maladaptive core beliefs, consideration of behavioural contingencies that developed from adverse childhood experiences, psychoeducation on attachment relationships and bonds, and exploration of the foundations of psychological challenges, such as feelings of blame and shame. Anyone undertaking TLSW should have additional training and skills to undertake this type of work and should be competent in therapeutic approaches.

The young person’s parent or carer may be included in the process to support attachment relationships and gain a joint understanding of the young person’s perception of their history. A Life Story Coach (LSC) could work alongside TLSW to support the process outside of the therapy room, for example, a foster carer supporting further consideration of a topic or thought pattern. A Life Story Permanent Product (LSPP) may also be created, which includes the young person's work so that they can refer back to key messages and realisations following the therapy.

​The need for effective Life Story Work is increasingly being recognised as essential for care experienced young people to support their wellbeing, mental health and integration into the family with which they live. Policy and practice guidance states that it should be integrated into care planning, carer training and practice. Many organisations have however found it difficult to effectively integrate Life Story Work into practice and support skill and confidence in its delivery. For many young people who have complex histories and information gaps regarding their family or history, LSDW provides structured support that can aid their understanding and support their emotional well-being. There is increased recognition that more Social Workers should receive specific training to support young people to navigate their lives and histories.
